Before your visit, please complete the new client paperwork

Your visit will be virtual via video conference and will be an 40 minutes long. During this time, you will have an evaluation, educational information provided, and then an individualized plan of care with you and the team will be completed.  

Labs will be ordered if no labs have been done in the last 3 months to assess for medical causes of weight gain/prevention of loss. If you have labs or EKG, then please fax them to 804-203-0806 before your visit. 

An EKG will be ordered if necessary 

Please note that the Virginia Board of Medicine requires that lab work including thyroid functioning be performed before starting weight loss medication. They also require an EKG for people who desire stimulating weight loss medication within 90 days of prescribing. If you have had lab work or an EKG, please fax this before you visit 804-203-0806.
